Tirzepatide is a 39-amino-acid synthetic peptide engineered to act as a co-agonist at two distinct incretin-hormone receptors simultaneously. The two receptor targets are the receptor for glucose-dependent insulinotropic polypeptide (the GIP receptor) and the receptor for glucagon-like peptide 1 (the GLP-1 receptor). The molecule was designed by Eli Lilly to incorporate sequence elements from both native incretin hormones along with a fatty-acid sidechain that supports albumin binding and once-weekly subcutaneous dosing through extended plasma half-life. Pharmacological characterisation establishes the compound as a biased agonist — the GIP receptor engagement is conformationally distinct from native GIP and produces a different downstream signalling profile (PMID 32730231).

The compound is FDA-approved for type 2 diabetes and chronic weight management in adults with obesity. The dual-receptor mechanism produces glycemic and weight-reduction effects that exceed monotherapy with single-receptor GLP-1 agonists in head-to-head comparison.
